📋 Documents Required at the Time of Registration
- Proof of Age / Identity — Certified birth certificate, baptismal certificate, prior school records with affidavit, or letter from DCS if the student is in foster care
- Proof of Residency — Utility bill, lease or rental agreement, mortgage statement, AZ Driver's License, bank statement, payroll stub/W2, government letter, tribal enrollment, or consular ID card
- Withdrawal Form from previous school (optional, may be requested later)
- Immunization Records (optional at enrollment, required prior to attendance)
Please note: Parents and legal guardians are provided 30 days from enrollment to provide proof of identity and age. You do not need all documents to begin the application. Someone from our team will follow up within 2 business days of submission.

Parents and legal guardians are not required to provide documentation, other than residency documentation and basic information (such as name and date of birth) in order to complete their enrollment. A.R.S. §15-184(A). We shall enroll all eligible pupils that submit a timely application and shall not deny a student admission unless the number of applications exceeds the capacity or the student has a prior or pending expulsion from another school. A.R.S. §§15-184(A), 15-184(E) and 15-184(I). A charter school shall not limit admission based on ethnicity, national origin, gender, income level, disabling condition, proficiency in the English language or athletic ability. A.R.S. § 15-184.
